Animal Sighting Report Form (263kb pdf) - Report observations of manta rays, whale sharks, other sharks and rays, sea turtles, marine mammals, octopus, squid, birds, or spawning organisms of any kind.

Lionfish Sighting Report Form (379kb pdf) - Report sightings of invasive lionfish anywhere in the sanctuary.

TRIP REPORT FORMS

Voluntary Vessel Trip Report Form (268kb pdf) - Use this form to let sanctuary staff know the basics about your recent trip. Please submit this form within 7 days of each visit to the sanctuary.

Violations Report Form (208kb pdf) - Use this form to report suspected violations of sanctuary regulations.
